It has been almost 18 months since James Maddison tore his ACL at the end of the 2024/25 season, and yet Tottenham Hotspur still have not had the former Leicester City man healthy enought to even start a game, much less play a full 90 minutes.
The No. 10 returned to the team at the end of the 2025/26 Premier League season and looked pretty good off the bench, but despite having a full offseason to train and get fully fit, Maddison still has not started games for Spurs.
He only came off the bench in the preseason and then did so again on Matchday 1 of the 2026/27 Premier League campaign, entering the pitch in the second half. And Spurs fans have been very concerned with the fact that Maddison still has not started a game, with many speculating about a possible aggravation, which the star No. 10 was quick to deny on social media.

Well, now Tottenham Hotspur have a new injury worry with James Maddison, as even in that brief appearance off the bench against Brentford, he went down hard on his shoulder and suffered a new injury. Although it is not a hugely serious injury, as some wondered, it is still significant and will cost him a few games - at a minimum.
Because Alasdair Gold of Football.London reports that Maddison will be back to playing in "a couple of weeks" on this shoulder injury, stating that the issue is not as serious as the "initial worries" indicated and that it is "good news" overall.
But try telling that to Tottenham fans, who certainly are not seeing Maddison being out for weeks as good news. And given how many times with players like Rodrigo Bentancur, Mohammed Kudus, and, of course, Dejan Kulusevski in the past someone ended up with a serious injury after they were only expected to be out for a matter of a few weeks, the Lilywhites are not going to rest easy with Madders right now.
With Xavi Simons out for months with a torn ACL, Maddison being in the lineup is more important now than ever before, especially since Conor Gallagher is the replacement No. 10 with Wilson Odobert also out with an ACL tear at the end of the 2025/26 season. And judging by how horrible Gallagher was against Brentford, the drop off between him and Maddison is massive. Spurs need him back, and this injury is bad news and also unsettling, given how many injuries Spurs have had for months with a story starting like this.
